Non Health Related Concerns

The UTMB CMC Department of Quality Services assists in the processing of health related inquiries.

Inquiries involving non-health related concerns:

  • Security
  • Job assignment
  • Parole review
  • Early release

Inquiries From Active TDCJ Offenders

Each offender within the Texas Department of Criminal Justice system has a grievance process available for their use to process their healthcare inquiries. The goal is to resolve issues/ concerns involving healthcare at the lowest level and provide the fastest method of resolution. The following is a brief overview of the process: 

  • Offender submits a Sick Call Request (SCR) to the unit provider.
  • If the response to the SCR does not resolve the inquiry then the offender may submit an I-60 to the unit provider.
  • If the response to the I-60 continues to not resolve the inquiry then a Step 1 Grievance can be completed.

The I-60 and Step 1 Grievance are both processes that occur at the unit level.

If the response to the Step 1 Grievance does not resolve the inquiry then a Step 2 Grievance can be completed. The Step 2 grievances are processed through the Central Grievance office in Huntsville thus allowing a review by Non UTMB personnel.

Inquiry From Parties Not Related to the Offender or Those From an Unknown Source

We redirect these inquiries to the offender for his or her use of the unit-based informal resolution or grievance process. We recommend you contact the TDCJ Health Services Division hotline at (936) 437-4271 or email your inquiry to the Texas Department of Criminal Justice Ombudsman Program via the following link: ombudsman@tdcj.state.tx.us.

Inquiry From Relatives or Family Members of the Offender

We, the Department of Quality Services, take our commitment to patient confidentiality seriously and for this reason we have established the following process to answer health related inquiries from family member or relative:

Obtaining a release of information from the offender allowing the release of health related information to the relative.

Due to HIPPA regulation we are unable to process inquiries via email. It is for this reason that we ask that you please complete the Health Care Inquiry Form.

Mail the completed form to:  UTMB – Department of Quality Services, 301 University Blvd., Galveston, Texas 77555-1007.
